1. Background:
The people in Mid-Western Region of Nepal are suffering from a diarrhea epidemic. According to the media reports, the death toll has risen to more than 200. Although some efforts are underway to control the epidemic, there is still a lot that needs to be done, and the epidemic, which started from the district of Jajarkot has spread to 11 more surrounding districts. Government and INGO reports show that there is a big gap in mobilizing health workers and volunteers who can conduct awareness campaigns locally on preventive measures against the disease.
In order to respond to the epidemic various civil society and youth groups initiated a campaign called “Paschim Paaila” or “Mission West” in the mid-July. The campaign was started with an aim to send trained youth volunteers with essential supplies to launch awareness campaigns on safe water, sanitation and hygiene, and distribute emergency materials such as chlorine solution for water purification and oral rehydration salts (ORS). This is a voluntary campaign led by Nepalese youth to help fellow Nepalese suffering from diarrhea in the Mid-West.
2. Day 1 (23 July, 2009)
Altogether 36 youth volunteers were ready to move to Jajarkot and Rukum –two most diarrhea affected districts. In depth training and orientation on objectives, activities and preventive measures that should be undertaken at the field was provided to these youth volunteers. Distinguished guests and representatives from various agencies such as UN HABITAT, WaterAid, ENPHO and MaHa Jodi were presented during see off programme organized at Maiti Ghar Mandala. Then we collected huge quantity of pamphlets from Department of Health Services at Teku that should be dispatched at nine districts of Mid Western region. I was nominated as Group Leader to organize the team during travel. The team left from Kathmandu at around 8:30 PM
3. Day 2 (24 July, 2009)
After about 20 hours of traveling, we have arrived Salli Bazar, Salyan at 3:15 PM. The Rapid Assessment Team (RAT) welcomed us and had managed accommodation for whole team. The orientation and discussion began after few hours of rest. RAT team briefed us about the ground situation and other logistics arrangement. Then the group division was made for Jajarkot and Rukum district (The list of group division is provided in Annex: 1). The logistics arrangements were distributed according to the number of people within the group.
4. Day 3 (25 July, 2009)
At around 5:30 AM the volunteers headed towards their destination. Then we travelled to Surkhet. After reaching at Surkhet, we headed towards “Regional Water supply and Sanitation Office” to meet with Divisional Engineer Mr. Prem Shrestha. During the meeting, we briefed him about our mission, progress and future activities. He suggested mobilizing few volunteers in vulnerable VDCs of Surkhet for performing preventive activities.













5. Day 4 (26 July, 2009)
In the morning, we went to the government’s control room established at “Regional Health Directorate” to meet with Dr. LK Pathak, Coordinator for controlling epidemic at Mid Western region. We briefed him about our activities, progresses and asked his permission for mobilizing more volunteers at Dailekh and Surkhet districts. He agreed and suggested to mobilize 15-20 volunteers in cooridination with Department of Public Health Offices at Dailekh and Surkhet. He also provided details of contact person at Dailekh.
Then we headed towards Dailekh district. After more than 8 hours driving, we reached at Dailekh. The coordination meeting was held with Dr. Guna Raj Agasti, Chief, Department of Public Health Office and he briefed about the epidemic situation and coordination mechanism at Dailekh. He told us about the coordination and sharing meeting on epidemic control with local stakeholders and journalists on the very next day i.e. on 27 July, 2009. He also requested us to prepare and share presentation on water quality and health including drinking water treatment options & preventive measures during diarrheal epidemic situations. Then we started preparing presentations on water & health.
6. Day 5 (27 July, 2009)
The sharing and coordination gathering started around 8:30 AM at DPHO meeting hall. There were more than 50 participants from various government agencies, I/NGOs, media. The meeting was chaired by Chief District Officer at Dailekh district. Dr. Agasti and his team made presentations on current diarrheal situation, status of mortality and mobility at Dailekh, mechanism of coordination & reporting system, mobilization of health staffs at affected communities. According to DPHO report, only 5 people have died due to diarrhea during the epidemic and out of that one death has confirmed from food poisoning. Altogether 2018 people were treated during health camps and 6339 people were treated for diarrhea, dysentery at local level by mobilized health professionals. Overall, there was very good mechanism of case management, treatment, staff mobilization and reporting system at Dailekh district.
Then I made presentation which included following slides on safe water, water quality & health, status of access to drinking water & sanitation at Mid West region including Dailekh, Household Water Treatment (HWT) options, chlorination of water sources, and preventive measures during epidemic. The presentation was followed by discussions and questions & answers sessions. Several queried raised by participants were addressed. Then, our team handed over 150 PIYUSH, 35 liter of chlorine solution, several IEC materials on HWT options & hand washing to Dr. Agasti. Dr. Agasti appreciated us for good informative presentation and useful materials, and has shown his commitment to distribute those materials at the grass root level through the DPHO team. He also requested us to send some volunteers to support the ongoing campaign at several diarrhea affected communities at Dailekh.
After the completion of programme, we headed back to Surkhet at around 12:30 PM. After arriving at Surkhet we met with Dr. LK Pathak and Mr. Prem Shrestha to brief our visit at Dailekh. Then we called at Kathmandu for preparing to send more youth volunteers at Surkhet and Dailekh.

7. Day 6 (28 July, 2009)
In the morning, we went to District Public Health Office, Surkhet and met with the chief. We discussed on mobilizing youth volunteers at selected VDCs at Surkhet and support from DPHO in this regard. Several materials such as PIYUSH, SODIS bottles, IEC materials were handed over to the chief to distribute at field during volunteer mobilization.
At around 10:30 AM, we prepared to return back to Kathmandu. During the traveling, we dropped Mr. Raju Shrestha at Chinchu who will move to Salli Bazar for further coordination with mobilized volunteers at Jajarkot and Rukum. Mr. Raju was given responsibility of coordinating at field and managing the volunteers mobilized for Dailekh and Surkhet. We reached Narayanghat (transit to Kathmandu) at around 9:00 PM and spent the whole night there.
8. Day 7 (29 July, 2009)
Early in the morning, we moved to Kathmandu from Narayanghat. As soon as we reached Kathmandu, we provided orientation to group of 22 volunteers ready to go to Dailekh and Surkhet. We explained them what we experienced during field visits and provided with all the information including contact details of persons at district level. Then the group headed to Surkhet with Mr. Ashish Singh from ENPHO as group leader.
9. Conclusion
Overall the field visit was very useful. Altogether 36 youth volunteers were trained and mobilized at Jajrkot and Rukum districts to response for diarrheal epidemic. Similarly, 22 more youth volunteers were provided with intensive training to move towards Dailekh and SUrket districts. During the field visits, good coordination was made with relevant stakeholders and the progress of Paschim Paaila activities were being also shared among the stakeholders.
In the whole, Paschim Paaila campaign was found very effective to response immediately for epidemic at Mid Western Region.

Second Group of Volunteers Departs to Surkhet

23 more volunteers left for Surkhet today in response to the call made by the assessment team at Surkhet. They received a 4 hour preparedness training before departure. Most of the volunteers will scatter in small groups around Surkhet’s VDCs and a few will work at Dailekh.
